Are you a dedicated and compassionate Nurse Practitioner looking to make a real difference in primary care? We are seeking an experienced and proactive Nurse Practitioner to join our supportive and forward-thinking GP surgery team.
About the Role:
In this rewarding role, you will work autonomously and as part of a dynamic multidisciplinary team to deliver high-quality, person-centred care to our patients. Key responsibilities include:
Providing advanced clinical assessments, diagnoses, and treatment plans for patients with acute conditions
Undertaking independent consultations, prescribing in line with policy and local guidelines, and making appropriate referrals
Supporting the proactive management of chronic disease patients in line with QOF and best practice
Participating in clinical audits, quality improvement projects, and regular MDT meetings

Strong collaborative skills to work alongside GPs, nurses, pharmacists, social prescribers, and administrative staff
A commitment to maintaining professional standards and continuing clinical development
